Their meta-analysis included data from 35 randomized trials assessing the effect of exercise on HDL levels in adults. While exercise regimens varied among these studies, on average patients in these studies exercised for 40 minutes, three to four times per week, and the effect on HDL was measured after eight to 27 weeks. &lt;/p&gt;    &lt;p&gt;Across the studies, participants had increases in HDL cholesterol averaging about 2.5 mg/dL. This increase in HDL cholesterol was only modest, but was statistically significant. Furthermore, since cardiac risk is thought to drop by two to three percent for each 1 mg/dL increase in HDL, a 2.5 mg/dL rise in HDL amounts to a substantial reduction in risk. &lt;/p&gt;    &lt;p&gt;Perhaps the most interesting finding from this study is the observation that the duration of exercise sessions - and not the frequency or intensity of exercise - correlated the best with rises in HDL levels. Evidence is building that with intensive medical therapy -- largely based on statins but also including aggressive risk-factor modification -- CAD can be halted or even reversed, and plaques can be &amp;quot;stabilized&amp;quot; to reduce the odds that they will rupture. In these individuals, exercise, smoking cessation, weight loss, blood pressure control and cholesterol control (aiming for LDL cholesterol levels of 100 mg/dL and HDL levels of at least 40 mg/dl) is especially important. &lt;/p&gt;    &lt;p&gt;The key, then, is to decide whether an individual is likely to have active CAD, that is, whether plaques are likely to be present, and then direct therapy accordingly. To a large extent, deciding whether plaques are likely to be present can be accomplished noninvasively. Begin with a simple assessment of risk to decide whether your risk is low, intermediate or high. ( &lt;a href="http://heartdisease.about.com/od/assessyourrisk/a/assessrisk.htm"&gt;Here's how to assess your risk simply and easily&lt;/a&gt;.) People in the low-risk categories probably need no further intervention. People in the high-risk categories should be treated aggressively (with statins and risk-factor modification), as they are very likely to have plaques. People in the intermediate risk category should consider noninvasive testing with &lt;a href="http://heartdisease.about.com/cs/cardiactests/a/EBT.htm"&gt;EBT scanning (calcium scans)&lt;/a&gt;: if calcium deposits are present on the coronary arteries, then they have plaques and should be treated aggressively. &lt;/p&gt;    &lt;h5&gt;When should blockages be looked for?&lt;/h5&gt;    &lt;p&gt;Blockages in the coronary arteries are still important. Most experts think that people in the high-risk category should have a &lt;a href="http://heartdisease.about.com/cs/cardiactests/a/thallium.htm"&gt;stress thallium test&lt;/a&gt;. If this test is suggestive of a major blockage, cardiac catheterization should be considered. A stress test or cardiac catheterization should also be strongly considered in anybody (whatever their apparent level of risk) who has symptoms of angina. Relieving blockages by surgery or stenting can be extremely effective in treating angina and, in some circumstances, can improve survival. &lt;/p&gt;    &lt;h5&gt;Summary&lt;/h5&gt;    &lt;p&gt;Our thinking about CAD has changed significantly over the past decade or so. It is not simply a disease of blockages that ought to be treated with stents. While some were half expecting the sponsors to pull a rabbit out of the hat at the last minute, and show that &lt;a href="http://drugsaz.about.com/od/drugs/vytorin.htm"&gt;Vytorin&lt;/a&gt; actually does provide some measurable benefit to patients with high cholesterol levels, that did not happen. &lt;/p&gt;    &lt;p&gt;There were no surprises of note. &lt;/p&gt;    &lt;p&gt;As I have &lt;a href="http://heartdisease.about.com/od/cholesteroltriglycerides/a/ENHANCE.htm"&gt;previously described&lt;/a&gt;, despite the significant reduction in LDL cholesterol in study patients who took Vytorin (a combination of ezetimibe and simvastatin) as compared to patients who took generic simvastatin alone, there was no benefit in the endpoint measured. (That endpoint, a measure of wall thickness in the carotid artery, generally reflects the amount of coronary artery disease that is present.) TGF-beta serves many important functions in the body; in the heart, it protects the aorta and other vessels from damage caused by a variety of factors, including hypertension and high blood cholesterol levels.&lt;/em&gt;&lt;/p&gt;&lt;br /&gt;&lt;em&gt;&lt;br /&gt;Cholesterol, however, suppresses the responsiveness of cardiovascular cells to TGF-beta and its protective qualities - thus allowing atherosclerosis to develop. Similarly, the research found that statins, drugs that lower cholesterol levels, enhance the responsiveness of cardiovascular cells to the protective actions of TGF-beta, thus helping prevent the development of atherosclerosis and heart disease&lt;br /&gt;&lt;/em&gt;&lt;br /&gt;        &lt;p&gt;&lt;em&gt;Dr. Huang believes that this research could lead to the development of novel and effective therapies to treat or prevent atherosclerosis. They found that changes in the DNA on the chromosomes were responsible for increased risk of developing coronary artery disease and heart attacks. However, it remains unclear whether it is heart cells that are doing the repair, or cells from elsewhere in the body.&lt;/p&gt;&lt;br /&gt;    &lt;p&gt;Richard Lee of the Harvard Medical School in Boston and colleagues genetically engineered mice so their heart muscle cells could be stained with a fluorescent protein.&lt;/p&gt;&lt;br /&gt;    &lt;p&gt;Around 80 per cent of the heart muscle cells in young mice picked up the stain.    &lt;blockquote&gt;&lt;br /&gt;        &lt;p&gt;The FDA made an exception and permitted the use of the Berlin Heart pump, which is approved for use throughout Europe but not the United States, to keep 5-year-old Joseph Greenwood alive as he awaited a heart transplant.&lt;/p&gt;&lt;br /&gt;    &lt;/blockquote&gt;&lt;br /&gt;    &lt;p&gt;This is a heart wrenching story that introduced me to the Berlin Heart pump.  It seems to be able to keep patients alive for up to several months while waiting for a transplant.  Sounds like a wonderful maching.&lt;br /&gt;&lt;/p&gt;&lt;br /&gt; But Jason, whose own heart failed last June, wouldn't have lived long enough to accept the transplant without the assistance of a mechanical external pump known as the Berlin Heart. The device kept Jason alive for 234 days at Lucile Packard Children's Hospital, longer than any other child in North America. Researchers from Children’s Hospital Oakland Research Institute, the University of Iowa and Roche Molecular Systems held a variant of the gene Leukotriene C4 Synthase (LTC4S) responsible on the basis of a study that began in 1971 and involved 11,377 participants. Various risk factors like weight, cholesterol levels and blood pressure were recorded and analyzed. They found that when those with a variant of gene LTC4S suffer an injury in their blood vessel, there is excessive inflammation, which hampers the repair of the blood vessel. This could lead to heart disease. The researchers are optimistic about their findings making personalized treatments possible."&lt;/em&gt;&lt;br /&gt;&lt;strong&gt;&lt;/strong&gt;&lt;br /&gt;&lt;strong&gt;Personalized medicine is on the way. The latest candidate is interesterified fats. These fats are produced by taking a type of saturated fatty acid that is considered relatively safe (stearic acid, present in chocolate), and combining it with vegetable oils (which contain unsaturated fat). The combination produces a type of fat that has the stability and solidity of saturated fats, but (it is hoped) the health profile of unsaturated fats. Indeed, interesterified fats are showing up in foods already. The Food and Drug Administration recently ruled that food companies can label products containing these new fats as 'high stearate' or 'stearic rich' fats, or as 'interesterified fats,' thus avoiding the politically negative buzz word, 'hydrogenated,' which is associated with transfats. "&lt;br /&gt;&lt;br /&gt;The new word is &lt;strong&gt;interesterified&lt;/strong&gt; . That will be showing up on labels.
